- Complete the Intake Paperwork
- Once you’re ready to move forward, I’ll send you the intake forms to complete.
- Schedule the Evaluation
No doctor referral is needed to initiate an evaluation unless you plan to submit for insurance reimbursement through a superbill. If your doctor does provide a referral, they can simply email it directly to me at parisa@schoolkidsOT.com
